Question:

All my tulips died last season, although they did fine the previous two years. Only some late-planted bulbs bloomed. Why?

Answer:

Some tulip bulbs just won't survive more than a couple of years, regardless of conditions. It could be that something got at the bulbs, such as moles or mice. Or some of your bulbs may have rotted because of poor drainage. One way to control critters is to put gravel or fresh kitty litter around the bulbs after planting.
